Gabapentin enacarbil available under the trade name Horizant is the only gabapentin product approved for treatment of Restless Legs Syndrome (RLS). Few patients with neuropathy experience clinically relevant benefit from any intervention (30%-48% with active and 11%-30% with placebo). 1 This JAMA Clinical Evidence Synopsis summarizes an updated Cochrane review on gabapentin for neuropathic pain. 2 Horizant (gabapentin enacarbil) is an extended release tablet used to treat restless legs syndrome and for the pain from having shingles (postherpetic nerve pain). Generic brands of gabapentin capsules, USP are used for postherpetic nerve pain and for add on therapy for partial onset seizures in patients 3 years and older. Warnings A Cochrane review of gabapentin for chronic neuropathic pain in adults confirmed that gabapentin is associated with greater rates of pain relief compared with placebo in post-herpetic neuralgia and diabetic peripheral neuropathy, but it concluded that evidence for other neuropathic pain conditions was weak . Physicians frequently prescribe gabapentin (Neurontin) or pregabalin (Lyrica) for this condition with mixed results. Alpha-lipoic acid has been studied and found beneficial for alleviating diabetic neuropathy ( European Journal of Endocrinology , Oct. 2012 ; Current Medical Research and Opinion , Aug. 17, 2018 ). CBD, CBG, and THC all have potential benefits for nerve pain, however, CBG, in particular, is of interest to scientists because it acts as an inhibitor of the neurotransmitter GABA, just as nerve pain medications like gabapentin do. The purpose of this report is to review the clinical evidence on the efficacy, safety and guidelines for use of gabapentin in adults with neuropathic pain, and to examine evidence on the misuse or abuse of gabapentin and other drugs for neuropathic pain.
