Three million U.S. citizens and 16 million people worldwide have or had an opioid use disorder. Between 1999-2019, over 500,000 deaths in the U.S. involved an opioid overdose. Design, Setting, and Participants Adults undergoing head and neck mucosal surgery from July 25, 2016, through June 19, 2017, were included in this double-blinded, placebo-controlled randomized clinical trial and randomized to receive gabapentin, 300 mg twice daily, or placebo before surgery and up to 72 hours after surgery. RESULTS: Postoperative gabapentin, a cortical neuronal calcium channel modulator, showed significant correlation with study variables. PubMed Central (PMC) Patients chose laparoscopic or open surgery and were then randomized to receive gabapentin 300 mg before surgery, then three times daily for 6 doses or placebo. There were 50 patients randomized to both the gabapentin and placebo groups for a total of 100 patients. Gabapentin and the related, more potent compound pregabalin have been shown to be beneficial in the treatment of neuropathic pain as well as postoperative pain following spinal surgery and hysterectomy. Postoperative gabapentin was associated with a significant 59.8 mg decrease in postoperative MME use, 21% in self-reported pain, and a 2.5 fewer doses of antiemetics administered but increased time to ambulation by 0.89 days. Multimodal pain management is proven effective in providing comfort and minimizing risk after surgery. We recommend several strategies, including: 1. Rest—Respect post-operative activity restrictions to avoid pain. 2. Non-opioid Pain Medication—Acetaminophen (Tylenol), Ibuprofen (Motrin, Advil), Aspirin, Mobic, Celebrex, Naproxen Gabapentin has also been shown to reduce immediate postoperative pain following mastectomies and other breast surgeries, and is often a component of ERAS protocols. 35,36 In their randomized control trial, Fassoulaki et al found that combining perioperative gabapentin and local anesthesia led to a significant decrease in chronic pain 3 months We defined new postoperative gabapentin as fills for 7 days before surgery until 7 days after discharge.
