You should definitely speak with your doctor, so he/she can help you slowly wean off this medication. Finally got my doctor to switch me from 800mg 4x a day of gabapentin to 75mg 3x a day of Lyrica, will this be an equivalent dose? Doc said he’d titrate me up next month if needed but I’m worried I’ll be in withdrawal if the pregabalin isn’t equal to my gaba dose. There’s many people that actually prefer Gabapentin over Pregabalin both therapeutically and recreationally despite it being a “light beer” ;). While they have a similar MOA work they still produce different results. OP: the conversion of 300mg of Pregabalin would be approximately between 1501 – 1800mg of Gabapentin. Gabapentin @ 900mg is equal to 150 Pregabalin. However the chart shows a range of 600/900gpt =150mg Pregabalin. Meaning that if you go on a high dose of lyrica you should be able to tolerate a high dose of Gabapentin. I generally won’t go higher than 900mg TID. Diminishing returns, TD, etc. Usually trial Lyrica but that “conversion” chart Lyrica has on their website is garbage. Snows every patient I try to to 1:1 with it. I generally just start over at a low dose and work my way back up. Not really a fan of either medication. Conversion between Lyrica and gabapentin is generally well tolerated and direct switching minimizes potential for gaps in pain relief. In the absence of seizure history, the drugs can be directly interchanged; patients can be advised to discontinue Lyrica and begin gabapentin the following day. Using the 6:1 ratio an equivalent dose of gapapentinoid can be determined using a step-wise approach. Determine total dose (in mg) taken by an individual in a 24-hour period. Divide total daily dose (in mg) of gabapentin by 6 to give total daily dose of pregabalin (in mg).
