Often known for its brand names Horizant, Neurontin, or Gralise, Gabapentin (GBT) is an anticonvulsant drug, but it’s sometimes used for migraine and headache prevention. Its original use is for seizure prevention for patients with epilepsy, or to alleviate the uncomfortable symptoms of shingles. Gabapentin for Migraine Prevention Initial dose: 300 mg once daily, with gradual increases as needed. Maintenance dose: 900-2400 mg per day, divided into three doses. While gabapentin can be a helpful adjunct therapy at lower doses, if it is used as a primary treatment, it often requires very high doses, which can become sedating. Gabapentin has a wide range of off-label applications, including as a treatment option for neuropathic pain, migraine prevention, and headaches, including tension and cluster types. Gabapentin (Neurontin) does not reduce the frequency of migraine headaches and is not recommended for prophylactic therapy, according to a systematic review of five studies. The review found no benefit of gabapentin or its prodrug gabapentin enacarbil compared with placebo, and reported common adverse effects.
