If you’re taking one of these medications more than once daily and you are tapering off it’s best to work slow at getting your doses equal. Then once that’s done you can start tapering off all doses at the same time. You can get liquid Lyrica for tapering or you can make it yourself. Both cause dependence From the few studies out there, the transition could theoretically be achieved by 2 approaches: 1) immediately stopping gaba and switching to lyrica OR 2) slowly titrating down 50% of the gabapentin dosage and starting with 50% of the desired pregabalin for few days, followed by eventually stopping gaba all together. 1. Cross-taper: A pharmacokinetic simu-lation model compared a stop/start approach with a four-day cross-taper whereby 50% of the gabapentin dose An in-house pharmacokinetic simulation suggests that a regime of halving the original gabapentin dose, and introducing half the intended dose of pregabalin on day 0, then stopping gabapentin and doubling the pregabalin dose on day 4 leads to fairly stable drug levels of pregabalin equivalents. I'd suggest checking with a pharmacist or at least asking your psychiatrist if you could potentially taper off from Gabapentin either while starting a low dose of Lyrica (then going up if needed) or tapering from Gabapentin first. Using the 6:1 ratio an equivalent dose of gapapentinoid can be determined using a step-wise approach. Determine total dose (in mg) taken by an individual in a 24-hour period. Divide total daily dose (in mg) of gabapentin by 6 to give total daily dose of pregabalin (in mg). Conversion between Lyrica and gabapentin is generally well tolerated and direct switching minimizes potential for gaps in pain relief.
