After taking a dose, IR gabapentin starts to work in the body within two to three hours. However, the full effects of gabapentin can take one to two weeks to become noticeable, and some people may need to wait longer to experience significant pain reduction. A typical gabapentin dosage is anywhere from 300 mg to 900 mg, 3 times a day. Some clinical studies have used dosages up to 1,200 mg , 3 times a day. There are a few different gabapentin products. In adults with postherpetic neuralgia, gabapentin capsules may be initiated on Day 1 as a single 300 mg dose, on Day 2 as 600 mg/day (300 mg two times a day), and on Day 3 as 900 mg/day (300 mg three times a day). The dose can subsequently be titrated up as needed for pain relief to a dose of 1,800 mg/day (600 mg three times a day). It is available in various forms, including capsules, tablets, and oral solution.While gabapentin's exact mechanism of action is not fully understood, it is believed to work by reducing abnormal electrical activity in the brain and modulating the effect of calcium channels. This helps to alleviate seizures and reduce nerve pain. Gabapentin requires three times daily administration because of its short duration of effect. Gabapentin enacarbil (brand name Horizant) only requires once-daily dosing. Only effective for partial-onset seizures, not other types of seizure disorders. Some branded and generic forms of gabapentin are not interchangeable. The time gabapentin takes to work is not the same for everyone.
