Hospitals usually give meds written for 3 x a day as 9am - 1pm - 5pm. However, at home you have more flexibility. I would consider the time you get up and the time you go to sleep. Renal adjustments are recommended in patients with CrCl below 60 mL/min. The usual dose to treat nerve pain in adults is 900mg to 3,600mg a day, split into 3 doses.
