Purpose: A case of probable gabapentin-induced visual hallucinations in a patient with no psychiatric history is reported. Summary: A 65-year-old white woman with no history of psychiatric Gabapentin can cause some rare but serious side effects. While these are uncommon, people should be aware of them and seek medical attention if needed. Gabapentin is also used to manage a condition called postherpetic neuralgia, which is pain that occurs after shingles. Gabapentin works in the brain to prevent seizures and relieve pain for certain conditions in the nervous system. It is not used for routine pain caused by minor injuries or arthritis. Gabapentin is an anticonvulsant. According to glutamatergic theories of schizophrenia, glutamate dysfunction, for example a significant reduction of glutamate levels caused by pregabalin could be the cause of hallucinations and other psychotic symptoms . More data is needed to understand the pathophysiology of pregabalin induced hallucinations and psychotic symptoms. Gabapentin, one of the antiepileptics, shows its effects via voltage-gated calcium channels. Sedation and mood elevation are among its side effects. High doses of gabapentin can cause autolytic ideation and auditory hallucinations, but stopping treatment resolves symptoms without recurrence. Gabapentin effectively improved both visual hallucinations and pain in a Parkinson's disease patient without adverse effects. In this article, we report four cases of hallucinations associated with sensory impairment, three visual hallucinations and one musical hallucination, that responded to gabapentin. To our knowledge, this is the only case of musical hallucinations successfully treated with gabapentin. Gabapentin was started in a dose of 300 mg bid, and sodium valproate was stopped. Within 48 hours, the patient developed new symptoms of hostility, verbal aggression, and command auditory hallucinations and destroyed property by breaking the commode in his room. The patient clearly attributed these new symptoms to gabapentin. Gabapentin is already on the list of drugs that can cause auditory hallucinations. Borobia-Perez reported about an 82-year-old woman already on gabapentin 300 mg/8 h for chronic pain. She developed psychosis, visual hallucinations, and worsened renal function after increasing the dosage of the drug to 600 mg/8 h and addition of 10 mg/day morphine.
