By Chris Faubel, M.D. Topical administration of medications for pain management has become increasingly more common. Pharmaceutical companies are getting in the game with products such as Flector patches, Voltaren gel, Pennsaid topical solution, Lidoderm patches, and Qutenza patches. Chronic NeP can markedly diminish both quality of life and daily function. Pain reduction is the main goal of treatment, but improving sleep, daily function, and quality-of-life are also important. 1 First-line medications include oral amitriptyline, gabapentin and pregabalin. The chapter concludes by identifying a set of active pharmaceutical ingredients (APIs) that are commonly used in formulations of compounded topical pain creams. This chapter opens with brief descriptions of the fundamentals of the practice of compounding in the United States. Gorman et al. showed that gabapentin compounded for topical use with other drugs degraded rapidly in their many active ingredient formulation. 10 Because compounding bases may affect drug stability, we investigated the stability and potency of gabapentin compounded as a single active ingredient formulation with Lipoderm cream, Versabase gel Find patient medical information for gabapentin oral and lidocaine-menthol topical on WebMD including its uses, side effects and safety, interactions, pictures, warnings and user ratings. Gabapentin - Topically administered gabapentin is reported to stabilize nerve endings thus decreasing nerve pain and helping to facilitate the healing process. Guaifenesin has been shown in several studies to be an effective muscle relaxant and has been used extensively in several European countries. To explore how the science of compounded topical pain creams affects their safety and effectiveness, this chapter begins with an overview of the art and science of compounding, which highlights the overall complexity involved in formulating safe and effective compounded preparations. In this study a variety of topical gabapentin formulations were investigated, including Carbopol ® hydrogels containing various permeation enhancers, and a range of proprietary bases including a compounded Lipoderm ® formulation; furthermore microneedle facilitated delivery was used as a positive control. Gabapentin is in a class of medications called anticonvulsants. What are the brand names of gabapentin? Gabapentin is available as both a brand name product and a generic product (chemically the same, usually lower cost than the brand name product). Brand names of gabapentin include Horizant®, Gralise® and Neurontin®. Bayview Pharmacy's Gabapentin 5%/Amitriptyline HCl 2%/Lidocaine 2% Topical Gel is a semisolid formulation dispensed through a pump mechanism. This design allows for easy and precise application on the skin, facilitating targeted treatment or absorption of active ingredients. Horizant (gabapentin enacarbil) is an extended release tablet used to treat restless legs syndrome and for the pain from having shingles (postherpetic nerve pain). Generic brands of gabapentin capsules, USP are used for postherpetic nerve pain and for add on therapy for partial onset seizures in patients 3 years and older. Warnings The Gabapentin 10%/Ketoprofen 20%/Lidocaine HCl 5% Topical Cream is a compounded medication that combines the effects of three active pharmaceutical ingredients (APIs) to provide relief from certain types of pain and inflammation. The active ingredient in this formulation is gabapentin, an anticonvulsant medication traditionally used to treat neuropathic pain, fibromyalgia, and other chronic pain conditions. When applied topically as a gel, it provides localized relief from pain and inflammation, making it an effective treatment for conditions like vulvodynia Gabapentin is an anti-epileptic medication that also diminishes pain transmission in damaged neurons through affecting glutamate receptors. Conditions that respond well to gabapentin cream or gel include: Postherpetic neuralgia; Diabetic neuropathy; Generalized vulvodynia; Fibromyalgia Formula Gabapentin 10% F 00Topical Gel (Suspension, 15 g) FIN 611 SUGGESTED PREPARATION (for 15 g) Weigh and / or measure the following ingredients when appropriate: The primary active ingredient in this formulation is Gabapentin, a well-known anticonvulsant. Gabapentin works by blocking certain nerve signals in the brain, which can help to reduce pain. Gabapentin may cause side effects such as dizziness, drowsiness, and dizziness. It is important to follow the prescribed dosage and seek medical attention if experiencing serious side effects or changes in mood or behavior. Gabapentin is prescribed by healthcare professionals and should only be taken under medical supervision. Gabapentin 40 mg/g Topical Cream is a specialized formulation designed to deliver the active ingredient, gabapentin, directly to the site of application on the skin. This semisolid preparation is dispensed through a pump mechanism, ensuring a convenient and controlled application. Known hypersensitivity to gabapentin or its ingredients (4) -----WARNINGS AND PRECAUTIONS----- Drug Reaction with Eosinophilia and Systemic Symptoms (Multiorgan ypersensitivity): Discontinue if alternative etiology is not established (5.1) •Anaphylaxis and Angioedema: Discontinue and evaluate patient In case reports by Safaeian et al. (2016) a topical cream with seven ingredients (diclofenac 5 percent, ibuprofen 3 percent, baclofen 2 percent, cyclobenzaprine 2 percent, bupivacaine 1 percent, gabapentin 6 percent, and pentoxifylline 1 percent), referred to as T7, was used for the treatment of radicular pain. The first patient was a 39-year
