Check your medicine each time you get a refill to make sure you receive the correct form. Neurontin (gabapentin) is used to treat pain you may have from shingles (postherpetic nerve pain). It is also used with other seizure medicines for partial onset seizures in patients 3 years and older. Depending on your response to gabapentin, your prescriber may slowly increase your dosage to 600 mg 3 times a day (for a total of 1,800 mg per day). The maximum dosage of gabapentin is 3,600 mg per day. Although the correct starting dose of gabapentin isn't agreed on, and is certainly variable per individual, it would be prudent to start on the lower end and increase to effect. Summary. Gabapentin is associated with a range of dose related side effects including dizziness, somnolence and nausea. It is often recommended to start at a low dose If you forget a dose, take it as soon as you remember. If it's within 2 hours of the next dose, it's better to leave out the missed dose and take your next dose as normal. Never take 2 doses at the same time. Never take an extra dose to make up for a forgotten one. If you have epilepsy, it's important to take this medicine regularly. • Agree the speed of dose reduction with the patient. • Typically, one change per week is recommended. Some patients will need space to acclimatise to the new dose so the dose changes may be every one to two weeks. Inform the patient that reduction can be slowed but not reversed. Gabapentin. How to take Gabapentin Gabapentin needs to be gradually increased over a period of time until a maximum daily dose of 600mgs be increased in 300mg increments every two to three days until tolerated. If you need to stop gabapentin, then follow the stepwise pattern in reverse to gradually decrease the dose each week or discuss with your doctor or spasticity team. Once you have reached step 7 you should continue on this dose until you see your GP or attend the Pain Clinic. If your pain is significantly improved before you reach step 7, you can stay on this dose. What if I have problems? For immediate-release gabapentin (Neurontin), dosing may be initiated with 300 mg on day 1, doubled on day 2 (300 mg twice a day), and tripled on day 3 (300 mg 3 times a day). The dose can then be titrated up as needed for pain relief to a maximum dose of 1,800 mg daily (divided into 3 daily doses). The risk of harm increases at doses above 120mg oral morphine equivalent (OME) daily. Above this dose the risk of harm and mortality increases substantially, but there is no increased benefit (Opioids Aware dose equivalents). Medication should only be continued when a person is demonstrating pain reduction and functional improvement. Dose TCA UP-TITRATION • Increase daily dose by 10 mg each week • Pain consultants advise a maximum daily dose of 30 to 40 mg if patient derives benefit with limited side effects. TCA DOWN-TITRATION / WITHDRAWAL • Reduce daily dose by 10 mg each week over a four week period. Dose for nocturnal sedation effect: 10 to 25 mg PO at night. Starting dose - Standard Dose Schedule Follow the instructions in the table below when first starting gabapentin. Once your pain has improved to an acceptable level, continue to take gabapentin at the dose you have reached. Gradually increasing the dose should lead to fewer side effects.
