Disclaimer: This generalized information is a limited summary of diagnosis, treatment, and/or medication information. It is not meant to be comprehensive and should be used as a tool to help the user understand and/or assess potential diagnostic and treatment options. The standard gabapentin titration schedule is as follow: the starting dosage is 300 mg and is increased by 300 mg/day, over the first 3 days, up to a total of 900 mg/day. This is increased by 400 mg/day from days 4 to 6 up to 1,200 mg/day to maximize efficacy and delivered three times a day (TID). NEURONATIN (GABAPENTIN) TITRATION INSTRUCTIONS Day 1 and Day 2: Take one pill at night Day 3: Take one pill 3 times a day Day 6: Take 2 pills 3 times a day Day 9: If you still have pain, take 3 pills 3 times a day *Remember to take this medication every day as instructed, this is not an “as needed” medication. Follow the instructions in the table below when first starting gabapentin. Once your pain has improved to an acceptable level, continue to take gabapentin at the dose you have reached. Gradually increasing the dose should lead to fewer side effects. Day Morning Afternoon Evening Day 1 300mg Day 2 300mg 300mg Day 3 300mg 300mg 300mg *If patient is unable to reach maximum effective dose of Gabapentin despite titration consider pregabalin. Pregabalin works on the same pathway as gabapentin. Evaluate people carefully for a history of drug abuse before prescribing gabapentin or pregabalin and observe them for development of signs of abuse and dependence Follow the table below taking from 1 tablet a day to a maximum of 2 tablets three times a day: Stay on three capsules a day for about a week and if your pain relief is adequate, keep on this dose. At this point, you should be taking 600mgs (2 capsules) three times a day. Continue on this dose until advised otherwise . Strict titration schedule: 12.5 mg day 1, then 12.5 mg BID days 2 & 3, then 25 mg BID on days 4 - 7, then 50 mg BID thereafter. NEURONTIN in patients 5 to 11 years of age is 25 mg/kg/day to 35 mg/kg/day, given in three divided doses. NEURONTIN may be administered as the oral solution, capsule, or tablet, or using combinations of these formulations. Dosages up to 50 mg/kg/day have been well tolerated in a long-term clinical study. .table_layout tbody td{ font-size:0.95em;} Usual Gabapentin Dosing (Adults) Usual initial gabapentin dose: 300mg q8h. Usual maintenance dose: 300-600mg q8h. Maximum dosage/day: 3600 mg Gabapentin Renal Dosing [>60 ml/min]: Give usual dosage : Dosage range: 400-1400mg/day (divided doses - Usually bid) : Dosage range: 200-700mg/day. : 100-300 mg/day. Use lower end of this range for CRCL Use gabapentin regularly in order to get the most benefit from it. It will take several weeks of regular use at an adequate dose in order to see changes in pain control. Gabapentin works best if it is taken at evenly spaced intervals throughout the day and night. dose until your follow up. c. Continue the titration schedule to the goal of 1800 mg/day of GABAPENTIN . d. You may stop titration early at the dose where you no longer have bothersome pain or at a pain level more acceptable for you. Not all patients will require the full schedule for pain relief. Titration of gabapentin to the maximum tolerated therapeutic dose is important. The therapeutic dosing range in neuropathic pain trials is 1800-3600 mg/day (normal renal function). The pharmacokinetics of gabapentin require regular dosing, it will not work if dosed “as needed.” Despite its therapeutic role in neuropathic pain, For daily doses of pregabalin below 150mg daily, e.g. 100mg, 75mg – switch to gabapentin 100mg tds, and titrate up if necessary. Renal impairment: dose reductions are recommended for both pregabalin (CrCl<60ml/min) and gabapentin (CrCl< 80ml/min). If appropriate to switch, please • If a patient develops acute pancreatitis, gabapentin discontinuation should be considered • History of substance abuse, psychotic illness. • Diabetes mellitus. Drug interactions • Gabapentin with opioids or TCAs: CNS depression. • Gabapentin with TCAs or duloxetine: increased risk of hyponatraemia Time to response: 2 weeks. Chronic pain is one of the most common reasons that patients seek medical attention. Chronic pain results from combined biologic, psychologic, and social factors, and most often requires a multifactorial approach to management. This leaflet mainly focuses on how to titrate gabapentin. For further information on gabapentin, please refer to the full patient information leaflet, which is included in every medicine package. Gabapentin should be avoided in pregnancy unless the benefit to the mother outweighs the risk to the fetus. Breastfeeding. Limited data indicate that gabapentin is excreted in low amounts in breastmilk. Maternal doses of gabapentin up to 2.1 grams daily produce relatively low levels in infant serum.
