Gabapentin enacarbil is used to treat moderate-to-severe primary Restless Legs Syndrome (RLS). RLS is a neurologic disorder that makes the legs feel uncomfortable. This results in an irresistible feeling of wanting to move your legs to make them comfortable. Gabapentin and gabapentin enacarbil are two commonly prescribed medications used to treat certain types of nerve pain and seizures. While both medications have similar names and belong to the same class of drugs, there are some key differences between them. Gabapentin enacarbil (Horizant (ER) (U.S. Tooltip United States), Regnite (in Japan)) is an anticonvulsant and analgesic drug of the gabapentinoid class, and a prodrug to gabapentin. [1] Gabapentin enacarbil is a transported prodrug of gabapentin that was developed to overcome the pharmacokinetic deficiencies of gabapentin. It has demonstrated efficacy and safety in treating Restless Leg Syndrome (RLS) in numerous clinical trials, with patients reporting sustained improvements in symptoms' severity. Gabapentin enacarbil (Horizant®) is a prodrug of gabapentin which binds with high affinity to the α2δ subunit of voltage-activated calcium channels in vitro studies. It is unknown how the binding of gabapentin enacarbil (GEn) to the α2δ subunit corresponds to the treatment of restless leg syndrome (RLS) symptoms. The conversion of gabapentin enacarbil to gabapentin after incubation for 1 h at 37°C in pancreatin and Caco-2 cell homogenate, rat plasma, human plasma, rat liver, and human liver was 42%, 75%, 25%, 5%, 71%, and 81%, respectively. Gabapentin enacarbil did not demonstrate any significant inhibition of the major isoforms of P450. Gabapentin enacarbil (Horizant) is a drug prescribed for the treatment of nerve damage from herpes zoster (postherpetic neuralgia, shingles), and restless leg syndrome. Side effects, drug interactions, and pregnancy safety information should be reviewed prior to taking any medication. The main advantage of Horizant is that it is a “pro-drug”of gabapentin.
