Gabapentin is not currently listed as a controlled substance under federal law. However, some states classify gabapentin as a Schedule V substance or a drug of concern and mandate reporting to PMP. Furthermore, other states are considering similar actions due to increasing evidence of associated risks. In the US, gabapentin was approved by the Food and Drug Administration as a non-controlled substance. To date, and in spite of empirical evidence suggestive of diversion and abuse with opioids, gabapentin remains a non-controlled substance at the federal level. Gabapentin is not currently listed as a controlled substance under the Controlled Substances Act of 1970.11 Several state boards of pharmacy, as outlined in Supplemental Table 2 and Figure 1, have independently reclassified gabapentin under state pharmacy rules as a Schedule V drug. Other states have required gabapentin use to be monitored The first marketed medication in this class, gabapentin, is not currently classified as a controlled substance in most states, however, its abuse potential is still being investigated. In fact, Kentucky, Michigan, and Tennessee have reclassified gabapentin as a Schedule V controlled substance.⁵˒⁶ Twelve states have not classified gabapentin as a controlled substance, but require gabapentin dispensing must be reported to their PMP (including CT, DC, IN, KS, MA, MN, NE, NJ, OH, OR, UT, and WY). Based on the authors’ findings, 22 states and federal districts had strengthened regulations on gabapentin. Of these, 68.2% enrolled gabapentin into their prescription drug monitoring program and 31.8% reclassified gabapentin as a Schedule V controlled substance (C-V). Two other states were in discussions on increased action on gabapentin misuse. However, because of increasing numbers of prescriptions and reports of misuse and harm, gabapentin is a controlled substance in some US states, with certain other states requiring gabapentin prescriptions to be reported to state databases. Gabapentin is not currently listed as a controlled substance under the Controlled Substances Act of 1970. Individuals at the highest risk for abusing gabapentin include those with opioid abuse, mental illness, or previous history of prescription drug abuse. However, several states consider gabapentin a Schedule V controlled substance.[3] In states where gabapentin is controlled, there are stricter laws for prescribing and dispensing it from pharmacies. There are five groups of controlled substances. Schedule V substances are the least likely of the controlled substances to cause addiction or To date, seven US states (AL, KY, MI, ND, TN, VA, and WV) have classified gabapentin as a controlled substance. In a petition filed with the DEA and the US Food and Drug Administration (FDA) in February, the nonprofit advocacy group Public Citizen called for making that regulation federal, in an effort to stop increasing abuse and diversion. Gabapentin isn’t a narcotic or federally controlled substance, but it is regulated and recognized as a controlled substance in certain states. This means it is not subject to the stringent regulations that apply to opioids or benzodiazepines, which are categorized based on their potential for abuse, medical use, and safety. Pregabalin is controlled in Schedule V of the Federal Controlled Substances Act . Although not controlled federally, some States also list gabapentin as a Schedule V controlled substance because of its abuse potential as a non-opioid pain reliever . iii At least 26 States and the District of Columbia have Effective July 1, 2018, all gabapentin products will be Schedule V controlled substances in the state of Tennessee. It is known under the brand names Neurontin, Horizant, Gralise, Gabarone, and Fanatrex. Gabapentin (Neurontin) is not a narcotic or federally controlled substance by the DEA as of November 2022, but it is classified as a Schedule V controlled substance in certain states. Gabapentin closely resembles pregabalin, a schedule V drug under the Controlled Substances Act in its chemical structure and pharmacological activity. The chemical structure of gabapentin is derived from the addition of a lipophilic cyclohexyl group to the backbone of gamma-aminobutyric acid (GABA). As of September 2022, gabapentin was classified as a controlled substance in Alabama, Kentucky, Michigan, North Dakota, Tennessee, Virginia, and West Virginia. 6,7 Adding gabapentin to the list of controlled substances has required providers to have a Drug Enforcement Administrationregistration number to prescribe it, adding another layer of
